Treatment & Reproductive Technology

• Artificial insemination (IUI)
• In vitro fertilization (IVF)
• Egg retrieval and embryo transfer
• Advanced reproductive technologies when medically necessary

Fertility Medications
• Oral and injectable fertility medications
• Medications used during fertility treatment cycles

Fertility Evaluation
• Physician consultation and medical history review
• Laboratory testing, imaging, and genetic screening
• Hormone and ovarian reserve testing
• Male fertility testing (including semen analysis)

Fertility Preservation
• Cryopreservation of eggs, sperm, embryos, or reproductive tissue
• Storage of preserved reproductive material

Important Reminders:
• Coverage levels and member cost sharing may vary by health plan.
• SB 729 coverage applies to Anthem HMO and Kaiser plans.
• SISC PPO plans do not include fertility benefits.

As SISC PPO plans are self-funded and not subject to California state regulations, those plans WILL NOT have fertility benefits beginning October 1, 2026.
