Medical Release from Physical Activity Due to Injury or Illness
Procedure for Medical Exemption:
If your child has sustained an injury or has an illness and he/she is unable to participate in physical activity, please follow these steps:

Obtain the Required Form: Obtain a copy of the "Medical Release for School Activity" or the "Physical Education Medical" form below. These forms are also in your school health office.
- Medical Release for School Activity form is for temporary PE excuses.
- Physical Education Medical form is used mainly at the high school level if your child will be out of physical education for 10 days or longer. Please contact your child's high school counselor.

Complete the Form: Fill out the form completely, providing details about your child's injury or illness and any specific restrictions or accommodations recommended by their healthcare provider.

Submit the Form to the School: Return the completed form to the school's health office. It's essential to ensure that all necessary information is included and that the form is signed by both the parent/guardian and the healthcare provider.

Review by School Staff: School staff will review the submitted form and make necessary accommodations for your child's participation in physical education or activities. This may include assigning alternative assignments or modifying activities as needed.
