2023-24 New Hire Benefit Packet

If your new position includes Health Benefits, please review the attached New Hire Employee Benefit Check Off List which lists documents to review and return. It is required that this check off list form be completed and returned to Risk Management with the Mandatory Documents.
 
Complete the mandatory documents shown on the check off list under the heading Action Required and return them to Viveka (Vee) Rodriguez in the Risk Management Department at PYLUSD, 1301 E. Orangethorpe Avenue, Placentia, CA 92870. If you have questions, please contact Vee at 714-985-8476. 
NOTE:
  • Employees working between 90% - 100% of a 1 FTE are required, per SISC rules, to enroll in a minimum of single medical coverage.
  • Employees working 50% to 89% of a 1 FTE may decline coverage (7.2 hours per day or 36 hours per week for classified staff)
  • Certificated and Management employees who work less than a 100% FTE pay a higher cost for their benefits. Call Risk Management for details.
  • All benefit documents must be received by Risk within 31 days of your hire date or life event or coverage is not guaranteed.
  • New classified, certificated, and confidential employees must work for PYLUSD for a total of two continuous years of employment (classified) or reach permanent employment status (certificated) to elect a PPO plan. This does not pertain to new management employees.
  • Certificated temporary employees (one-year contract) and temporary employees whose service days begin after the beginning of the second semester shall not be entitled to benefits beyond June 30th. 
For a smooth & timely enrollment, documents are required 10 working days from your date of hire. If documents are received after the 15th of the month, there will be a delay in your enrollment as there is a two-week enrollment process period.

Please note:
 
  • Effective 7/1/16 Cigna will no longer be available to new enrollees. 
  • Employees who are currently enrolled in the Cigna dental plan may elect to stay in the plan.
  • If you are hired prior to October 1, you will have additional documents to complete as the previous plan year is in effect until September 30. Contact Risk Management for instructions.
