Medicare Part D Notice

Dear Placentia-Yorba Linda Unified School District Employee/Retiree,

Federal law requires that employers who sponsor a health plan offering prescription drug benefits provide an annual notice to all Medicare-eligible participants that states whether the prescription drug benefits offered under the plan are creditable or non-creditable.

Placentia-Yorba Linda Unified School District is providing this notice to all employees. If you and/or your dependents are not Medicare eligible, you may disregard this notice.

The Medicare Part D notice must be distributed:

At least once a year before October 15 (the start of the annual Medicare Part D enrollment period).

Whenever a Medicare-eligible employee, spouse or dependent enrolls in a Placentia-Yorba Linda Unified School District sponsored health plan.

Whenever there is a change in the creditable or non-creditable status of Placentia-Yorba Linda Unified School District’s health plans’ prescription drug coverage.

Whenever an individual requests the notice.
