Keenan is the workers' compensation third-party administrator, and Company Nurse is the nurse reporting line. The number to call to report an injury is 866-227-7557.
If you are a PYLUSD employee and are injured at work, notify your supervisor immediately and follow the directions on the Company Nurse flyer attached below.
Your supervisor or their designee will supply you will all other necessary documents if you choose to file a workers' compensation claim.
Employees: If you would like to treat with your personal physician for a work-related injury, you must complete a Physician Pre-Designation form (see below). This form must be signed by your doctor, date-stamped, and approved by Risk Management before the reported injury.
