If you are a PYLUSD employee and are injured at work, notify your supervisor immediately and follow the directions on the Injury Nurse Advice flyer attached below.
Your supervisor or their designee will supply you will all other necessary documents if you choose to file a workers' compensation claim.
Employees: If you would like to treat with your personal physician for a work-related injury, you must complete a PYLUSD Pre-Designation Choice of Physician form (see below). This form must be signed by your doctor, date stamped, and approved by Risk Management prior to the reported injury.
