American Fidelity Assurance Company (AFA) is a District partner that offers voluntary benefits to employees with the convenience of payroll deductions. This includes disability, accident, cancer, policies, and life insurance options. The American Fidelity main office number is 800-662-1113.
 
** Disability Insurance** If employees receive a mid-year salary increase they are responsible for notifying AFA to increase their disability policy amount. This will ensure the correct amount can be claimed if a disability arises. Employees will be paid, while on disability, at the rate established during open enrollment or whenever AFA is notified by the employee a change has occurred. Note: Increases will exclude all pre-existing conditions until the pre-existing condition wait period is over, this includes pregnancy.
 
**Life Insurance with Long-Term Care Rider** As an alternative to long-term care plans, attached is a life insurance rider that offers from 25% up to 92% of the policy value when faced with a disabling chronic illness.
